How do I add metrics to the Screening Form?

NOTE: If you don't see a metric, contact [e-mail] to add one.

1. Select the Settings Tab
2. Click on the Screening Form Tab
3. Click on the Caret of the form you would like to edit
4. Drag and drop the metric you would like to add and move it from the right side to the left

     (i.e: The addition of the Vaccine Name shown below)
